Bram: Hé Mieke, goh, wat toevallig. Kom jij ook voor een afspraak?
Hey Mieke, oh, what a coincidence. Are you here for an appointment too?

Mieke: Nee, ik moest alleen even een formulier ophalen bij de assistente. En jij? Ben je ziek?
No, I just had to pick up a form from the assistant. And you? Are you sick?

Bram: Gelukkig niet. Maar ik hoorde net iemand aan de balie praten over een ‘doorverwijzing’. Dat is echt zo’n woord dat je alleen in een ziekenhuis of hier hoort, hè?
Fortunately not. But I just heard someone at the counter talking about a ‘referral’. That’s really one of those words you only hear in a hospital or here, right?

Mieke: Haha, ja, klopt. Maar het is wel een heel belangrijk woord om te kennen. Zeker voor het KNM-examen.
Haha, yes, that’s right. But it’s a very important word to know. Especially for the KNM exam.

Bram: Oh ja? Leg eens uit. Stel, ik heb al twee weken last van mijn rug. Dan wil ik toch gewoon naar een specialist in het ziekenhuis?
Oh really? Explain. Suppose I’ve had back pain for two weeks. Then I just want to go to a specialist in the hospital, right?

Mieke: Dat klinkt logisch, maar zo werkt het in Nederland niet. Je moet met je klacht altijd eerst naar de huisarts.
That sounds logical, but that’s not how it works in the Netherlands. You always have to go to the general practitioner first with your complaint.

Bram: Echt waar? Ook als je bijna zeker weet dat je een specialist nodig hebt? Dat klinkt niet zo efficiënt.
Really? Even if you’re almost certain you need a specialist? That doesn’t sound very efficient.

Mieke: Toch wel. De huisarts is de poortwachter van de zorg. Hij of zij bekijkt jouw klacht en beslist of een specialist nodig is. Zo gaan niet te veel mensen onnodig naar het dure ziekenhuis.
It is. The general practitioner is the gatekeeper of healthcare. He or she assesses your complaint and decides whether a specialist is needed. This way, not too many people go to the expensive hospital unnecessarily.

Bram: Oké, dus ik maak een afspraak hier. En dan?
Okay, so I make an appointment here. And then?

Mieke: Dan bespreek je de klacht. Als de huisarts denkt dat een specialist je moet zien, krijg je een doorverwijzing. Dat is de officiële toestemming.
Then you discuss the complaint. If the general practitioner thinks a specialist should see you, you get a referral. That is the official permission.

Bram: Ah. Ik herinner me dat van mijn oma. Zij wilde vorig jaar naar een oogarts, maar dat mocht niet zomaar van de zorgverzekering.
Ah. I remember that from my grandmother. She wanted to see an ophthalmologist last year, but the health insurance company wouldn’t just allow it.

Mieke: Precies. Ze moest eerst naar haar eigen huisarts. Pas nadat die had gezegd dat het nodig was, kon ze een afspraak maken bij de specialist.
Exactly. She first had to go to her own general practitioner. Only after he or she had said it was necessary could she make an appointment with the specialist.

Bram: En zonder die doorverwijzing wordt de behandeling in het ziekenhuis dus niet betaald?
And without that referral, the treatment in the hospital won’t be paid for?

Mieke: Inderdaad. De zorgverzekeraar betaalt de rekening van de specialist alleen als je een geldige doorverwijzing van de huisarts hebt.
Indeed. The health insurer only pays the specialist’s bill if you have a valid referral from the general practitioner.

Bram: Wacht even, voor de duidelijkheid. Je hebt het over een ‘doorverwijzing’ en soms over een ‘verwijsbrief’. Is dat hetzelfde?
Wait a minute, for clarity. You’re talking about a ‘referral’ and sometimes about a ‘referral letter’. Is that the same?

Mieke: Goede vraag. Ze horen bij elkaar. De ‘doorverwijzing’ is de handeling, de beslissing van de dokter. De ‘verwijsbrief’ is het document dat je krijgt.
Good question. They belong together. The ‘referral’ is the action, the doctor’s decision. The ‘referral letter’ is the document you receive.

Bram: Dus het papier of het digitale bericht.
So, the paper or the digital message.

Mieke: Precies. Met die verwijsbrief kun je de afspraak bij de specialist maken. Dus onthoud die twee woorden goed: de doorverwijzing en de verwijsbrief.
Exactly. With that referral letter, you can make the appointment with the specialist. So remember those two words well: the referral and the referral letter.

Bram: Helder.

Mieke: Wat belangrijk is om te onthouden voor je examen: ze stellen vaak een vraag over wie de doorverwijzing geeft. Dat is een typische KNM-vraag.
What’s important to remember for your exam: they often ask a question about who gives the referral. That’s a typical KNM question.

Bram: Oké, en wat is dan het foute antwoord dat veel mensen geven?
Okay, and what’s the wrong answer that many people give?

Mieke: Soms denken mensen dat de zorgverzekeraar dat doet, of dat je het zelf mag kiezen. Maar het juiste antwoord is altijd: de huisarts.
Sometimes people think the health insurer does that, or that you can choose it yourself. But the correct answer is always: the GP.

Bram: Oké, dus als ik op het examen een vraag krijg over wie een doorverwijzing naar een specialist geeft, dan is het antwoord ‘de huisarts’. Simpel eigenlijk.
Okay, so if I get a question on the exam about who gives a referral to a specialist, the answer is ’the GP’. Simple really.

Mieke: Precies. Stap één is altijd de huisarts. Zonder die stap kun je niet naar een specialist in het ziekenhuis voor een behandeling.
Exactly. Step one is always the GP. Without that step, you can’t go to a specialist in the hospital for treatment.
