Bram: Pfff, Mieke, wat duurt dit lang, zeg. We zitten hier nu al bijna een uur.
Pfff, Mieke, this is taking so long. We’ve been here for almost an hour now.

Mieke: Ja, de wachttijd op de spoedeisende hulp kan flink zijn. Hoe gaat het met je enkel? Doet het nog steeds zo’n pijn?
Yes, the waiting time at the emergency room can be considerable. How’s your ankle? Does it still hurt so much?

Bram: Ja, hij is dik en blauw. Ik verdraaide hem tijdens het voetballen. Ik dacht, ik ga wel even langs het ziekenhuis.
Yes, it’s swollen and blue. I twisted it while playing football. I thought I’d just pop by the hospital.

Mieke: Heb je wel eerst de huisartsenpost gebeld? Je kunt niet zomaar naar de spoedeisende hulp gaan.
Did you call the out-of-hours GP service first? You can’t just go to the emergency room.

Bram: Ja, natuurlijk. Het was na zessen, dus mijn eigen huisarts was dicht. Ik moest de huisartsenpost bellen. Zij zeiden dat ik hierheen moest komen, waarschijnlijk voor een foto.
Yes, of course. It was after six, so my own GP was closed. I had to call the out-of-hours GP service. They told me to come here, probably for an X-ray.

Mieke: Ah, kijk, dan heb je de juiste route gevolgd. De spoedeisende hulp, vaak afgekort als SEH, is echt alleen voor een noodgeval. Of dus met een verwijzing van de huisartsenpost.
Ah, see, then you followed the correct procedure. The emergency room, often abbreviated as ER, is really only for an emergency. Or with a referral from the out-of-hours GP service.

Bram: Ik zie hier ook wel mensen die er veel slechter aan toe zijn dan ik. Net kwam er een ambulance via die speciale ingang daar. Dat voelt toch wel serieus.
I also see people here who are much worse off than me. An ambulance just arrived through that special entrance over there. That feels quite serious.

Mieke: Zeker. Daarom moet je ook zo lang wachten. Er kwam net een verpleegkundige even bij je kijken, toch?
Exactly. That’s why you have to wait so long. A nurse just came to check on you, right?

Bram: Ja, heel kort. Ze stelde een paar vragen en keek even. Ze zei dat de arts eraan kwam als het rustiger was.
Yes, very briefly. She asked a few questions and took a look. She said the doctor would come when it was quieter.

Mieke: Dat heet triage. De verpleegkundige schat in hoe ernstig het is. Iemand met pijn op de borst gaat altijd voor op iemand met een zere enkel.
That’s called triage. The nurse assesses how serious it is. Someone with chest pain always takes precedence over someone with a sore ankle.

Bram: Logisch, haha. Dit is wel heel anders dan mijn afspraak van vorig jaar. Toen had ik last van mijn oog en moest ik naar een specialist.
Logical, haha. This is very different from my appointment last year. Back then, I had trouble with my eye and had to see a specialist.

Mieke: Dan moest je vast naar de polikliniek.
Then you must have gone to the outpatient clinic.

Bram: Ja, precies. Ik wilde meteen een afspraak, maar dat mocht niet. Ik moest eerst een verwijzing van de huisarts hebben. Uiteindelijk kon ik pas na vijf weken terecht.
Yes, exactly. I wanted an appointment right away, but I couldn’t. I first needed a referral from the GP. In the end, I could only go after five weeks.

Mieke: Dat is het grote verschil. Een polikliniek is voor geplande zorg, niet voor noodgevallen. Daar maak je een afspraak.
That’s the big difference. An outpatient clinic is for planned care, not for emergencies. You make an appointment there.

Bram: En wat als de arts straks zegt dat mijn enkel gebroken is en ik hier moet blijven?
And what if the doctor says my ankle is broken and I have to stay here?

Mieke: Als je in het ziekenhuis moet slapen, dan heet dat een opname. Maar laten we hopen dat dat niet nodig is.
If you have to stay overnight in the hospital, that’s called an admission. But let’s hope that’s not necessary.

Bram: Oké, dus even checken of ik het snap. De spoedeisende hulp is voor een noodgeval. En de polikliniek, wat is dat dan ook alweer precies?
Okay, so let me check if I understand. The emergency room is for an emergency. And the outpatient clinic, what exactly is that again?

Mieke: De polikliniek is de plek in het ziekenhuis waar je een afspraak hebt met een specialist, zoals een oogarts of een kinderarts. Dat is dus altijd gepland, nooit acuut. En je hebt een verwijzing nodig.
The outpatient clinic is the place in the hospital where you have an appointment with a specialist, such as an ophthalmologist or a pediatrician. So that’s always planned, never acute. And you need a referral.

Bram: Duidelijk. Spoedeisende hulp voor spoed, polikliniek voor een afspraak.
Clear. Emergency room for emergencies, outpatient clinic for an appointment.

Mieke: Precies. En dit is belangrijk voor je KNM-examen. Ze geven je vaak een korte situatie. Bijvoorbeeld: ‘U heeft al twee weken last van uw rug.’ Het antwoord is dan nooit ‘naar de spoedeisende hulp gaan’.
Exactly. And this is important for your KNM exam. They often give you a short scenario. For example: ‘You’ve had back pain for two weeks.’ The answer is never ‘go to the emergency room’.

Bram: Wat is het juiste antwoord dan?
What’s the correct answer then?

Mieke: Altijd eerst een afspraak maken bij de huisarts. Onthoud dat goed.
Always make an appointment with the GP first. Remember that well.

Bram: Oké, dus voor het examen moet ik weten: bij een acuut noodgeval, zoals een ongeluk, ga je naar de spoedeisende hulp, vaak na bellen met 112 of de huisartsenpost.
Okay, so for the exam I need to know: in an acute emergency, like an accident, you go to the emergency room, often after calling 112 or the GP out-of-hours service.
